Dental examinations are only carried out once the doctor has reviewed your medical history and other needs and concerns.

We have provided our new patients with online forms to fill out for ease and comfort. It’s advised to print out and complete your new patient form before your Pine Lake Family Dentistry appointment to avoid any inconvenience.
